For people who do not want physically invasive psychiatric treatment
To make this living will all that you need to do is to complete and submit the form on the next page. You will then be presented with a printable version of your living will. This is entirely free
Alternatively, for a small charge we will:
- Send you a credit card size version of your living will which you can keep (if you wish) in your wallet or purse
- Send you a full version of your living will.
- Send you a notice that you can send to your GP informing him or her of your living will.

Additional information
The Mental Capacity Act 2005 allows a person in England or Wales to make what is known as an "advance decision" to refuse treatment. This is a so-called "living will". If you sign your living will in front of a witness it will also extend your advance decision to life sustaining treatment, which will not be the case otherwise.
You should also be aware that, as the law stands at present, if somebody is compulsorily detained under the Mental Health Act, whilst they are so detained any advance decision they have made in relation to mental treatment must be taken into account, but can be overridden.
Data Protection: We will keep your information confidential and not pass it on to any third party without your consent. The only exception to this is if we are approached by a doctor, hospital or health authority seeking confirmation that you have made this living will, or we are otherwise asked to confirm to a doctor, hospital or health authority that you have made this living will. We will normally provide such confirmation.
You can change or cancel your living will at any time.
